Breast augmentation is one of the more popular procedures performed in aesthetic surgery. Natural results can be achieved and I believe that the larger one makes the breast, depending on ones skin and subcutaneous tissue and the amount of breast tissue already there, the more likely one compromises on a natural look. The more breast tissue one has, the more natural the look can be. I personally prefer the anatomical implants and honestly believe these give a more natural look than the round implants. I use the round implants to give a fuller upper pole which is more of an unnatural look. Deciding on whether or not a patient needs the implants placed under the breast or under the muscle, I prefer placing the implant under the breast in most circumstances except when the patient is very thin where there might be problems with implant edge show or implant wrinkling.
The sites of insertion can be in the inframammary crease, under the arm, or around the areola. I do all three but believe that the inframammary crease gives the best advantage regarding pocket adjustments, implant placement and hiding the scar. I create the pocket through diathermy dissection which is very precise and less traumatic than blunt dissection. I believe drains are important in draining the blood and serous fluid post-operatively. I believe that this has definite advantages.
